Chronic care management in Denmark

Overview
A fee for a type 2 diabetes management programme was introduced in Denmark following major health care reforms in 2007. The main objective of the scheme is to improve integration of care in chronic conditions and coordination of care by strengthening the role of general practitioners. The implementation period is still on-going and the incentive scheme has not been appropriately evaluated. The fragmentation of the Danish health care system and the lack of appropriate communication systems among providers pose some challenges to the programme’s success.WHO Team
